·         Please bring the following to each appointment so we can best serve your needs:

Ø      List of current medications including prescriptions, over the counter medicines, vitamins and supplements.

Ø      List of questions/concerns you would like to discuss with a member of the team.

·         Many times we can refill your prescription by phone.  This depends on the medication and when you were last seen. 

Ø      Before your prescription runs out call (517)853-0781 and leave a message with a nurse on our prescription line.


·         Please be prepared to pay (or co-pay) for services at the time of your appointment.

Ø      For your convenience, we accept check, Visa, MasterCard, Discover and cash.
